All video and scripts must be reviewed by two people.
For example, a producer writes and proofreads a script: review number one.
Then an anchor pre-reads the script: review number two. Both should initial that they've read the script. There should always be 2 initials.
The same applies for all video - two people must review it.
For example, a reporter should review his/her package when it is complete: review number one.
Then the editor takes in the package: review number two.
Producers can also check video (and should - especially headlines and teases.)
