The first image shows the declaration form (declaratieformulier) used at Kennisbank Filantropie. My input for this declaration form was adding the sum formula and slightly adjusting the lay-out. The second image is a translation of the first image. I had an English Excel course in mind. The third image shows a makeover in the Databeet style. And the last image is an example of how they all look when printed. It is an template to declare costs that can be customized to the corporate identity. It can be used by people who claim the prepaid expenses they made for the organization, by e-mail or by paper. This way, all the expenses employees need to claim to get their money back are processed in the same style. This speeds up the payment process.
