The surveys below are a reflection of a potential application that can be used as a foundation for screening potential Partners. The different areas are to prompt an understanding of the different organizations’ baselines and how they fit into the context of the Data Collaborative.

The Data Collaborative Partners (Direct Practitioners) participate in triads in the Data Collaborative community. Each organization has three individuals with unique and diverse sets of skills that, when put together, encompass the total body of work that the Data Collaborative's Aim requires. Each person has specific bodies of work that help fulfill the requirements of the work. To help guide and assist in selecting the right people, we have provided the following information for the positions reflected below. Each position below will include the following:

The survey below is to assess the content knowledge for the four different pillars. It will allow the organization facilitating the capacity development to understand how much understanding each participating individual has on a specific content area. With that information, they will be able to develop and place Participants in the appropriate training.
Below, please find a foundation to start building a memorandum of understanding. This is based on a three year timeline and the original project scope and sequence. We would suggest making amendments to the necessary requirements based on the scope and sequence that you are proposing. The scope and sequence would be based on the concepts that you are adopting, as well was the results of the baseline survey (which will help predict the level of content knowledge of the participating individuals).

Below are the Scope and Sequences that the Data Collaborative followed for Year 2 & Year 3. They are based on the different levels of supports needed for the Partners based on the different years and content.
