This form allows EXITO to distribute stipends during the academic year. Each stipend form is specific to a Scholar.

Please review all information on the form to ensure accurate information. If anything is incorrect please make the correction and then sign. 

• Participant (Vendor) ID Number: This should be your PSU ID

• Participant (Vendor) Name

• Total Payment to Participant: This should be $10,755

• Total Number of Payments: This should be 9

• Payment Amount: This should be $1,195

• The stipend form must be signed by 9/22 @ noon
