- Your Name is completely optional. This can be an anonymous survey if you wish, though I appreciate hearing from you and knowing who you are.
- The answer in this survey will not affect your grade in a positive or negative way. However, completion of the survey in its entirety is required.
- Some answers are short paragraphs and they require a minimum amount of words. Make sure you set aside about 20 minutes to answer the entire survey.
- Once you are done with the survey, SHOW THE VERIFICATION SCREEN to Mr. Sarabia to make sure you get your credit for the survey. If you close the screen without showing the verification you will have to fill out the survey again.
