Anytime you come across a book that needs to be looked at, or is damaged, fill out a Health Check form. These forms can be found in the reference / circulation area behind the circ desk (by the emergency action plan). Then, place the book in the Evening Supervisor's office. Be sure to fill out the form with as much information as is possible.
If the damage is superficial (torn pages, damaged covers, Fido chewed on the book, etc.) fill out one of these forms, place the form in the book (like a bookmark), and place in problem children bin in evening circulation supervisors office.
If the damage is NOT superficial (silverfish, mold, and water damage are the primary examples) then do NOT put it in the problem children bin OR on a supervisors desk. You are to retrieve a cart from the holding area (where the night book-drop is located), place the book and completed form on that cart, and then roll the cart into the Evening Circulation Supervisor's office. These books need to be isolated because this damage CAN SPREAD to other books and paperwork.
