This project anti-pattern is more of not executing proper testing and safety execution until there is an issue. The alternate terms is "wait until the management becomes desperate, and they will only accept what you give them".
Symptoms and Consequences
There are always "fire-fighting" activities across the event.
Project delayed due to ad-hoc fire-fighting activities.
Causes
Poor trust between development team and management.
Laziness from development team.
Resources constraints (e.g. not enough time).
Refactor Recipes
If the management is really toxic, use sheltering technique to protect the development team from toxic distraction and demotivation.
If the cause is from development team, have a common understanding and execute a fire-fighting prevention plan.
