An incident report is completed where any unscheduled situations result in;
Injury, illness or fatality to staff or participants
Significant alterations or modifications to the program or planned route
Emergency situations where evacuation or rescue actions are required
Any event that triggers an incident report, must be completed by staff within 48 hours of incident. The below button will take you through to a series of questions that must be completed. This task is MANDATORY.
This generally includes:
Reporting the accident – Notifying James or the safety officer.
Providing immediate care – First aid or medical attention.
Recording the incident – Completing an accident/incident report.
Investigating the cause – Identifying what led to the accident.
Implementing corrective actions – Measures to prevent recurrence.
Monitoring and review – Following up on staff recovery and effectiveness of preventive measures.

Hope you are well.
I know you only manage our Public Liability insurance, however I just wanted to inform you we had an injury with one of our casual staff members yesterday.
Declan Baker (one of our guides) - got his thumb caught in his abseil device and lost half of his thumb. He is an extremely experienced guide, and it appears to be a freak accident. It does not appear that he was not doing anything wrong.
Regarding Workers' Compensation, can you advise us on the correct next steps to handle the situation? Here is what we have done so far:
We administered first aid
We spoke to 2 doctors (orthopedic surgeon and head of ER - Nepean Hospital) and arranged for him to see the specialist hand unit at RNSH. A guide drove him straight there on advice from both doctors (who advised the local hospital didnt have the specialst care needed to look after him)
Declan has been sms-ing me updates. He is being treated at RNSH - and is expected to have surgery today.
