Trigger

An incident report is completed where any unscheduled situations result in;

• Injury, illness or fatality to staff or participants

• Significant alterations or modifications to the program or planned route

• Emergency situations where evacuation or rescue actions are required

All Staff

Any event that triggers an incident report, must be completed by staff within 48 hours of incident. The below button will take you through to a series of questions that must be completed. This task is MANDATORY.
