Standardized testing is used in the TSTC LVN to ADN Transition Program as part of the program assessment plan, as well as a tool for students to measure their progress and remediate in areas in which they may be deficient. All level 1 and 2 students enrolled in TSTC LVN to ADN Transition courses will take the nationally normed ATI Content Mastery Assessment Proctored Exams in each course. All level 3 students enrolled in TSTC LVN to ADN Transition courses will take the nationally normed HESI Specialty Exams in each course and the EXIT (E2) RN Exam in a course in the final semester of the program.
LEVEL 1 and LEVEL 2
All Level 1 and Level 2 students will take a version of the ATI Content Mastery Assessment exam at the start of each course. Students will take the ATI exams remotely or in a computer lab. All exams will be proctored using Proctorio or Respondus. It is highly recommended that the student reads all rationales, if available, once the ATI exam is completed. A score will be shown immediately after the exam is completed. This score will serve as a baseline and will help students determine areas of strength and areas that need improvement. Remediation for exams will be per the program's remediation policy. The purpose of remediation is to improve the student’s critical thinking, reasoning skills, and test taking strategies to achieve NCLEX-RN success.
A Remediation Plan and Outcomes form will be made available to the student. After identifying areas for remediation and completing the form, a signed copy will be placed in the student's file and serve as a contract between the student and instructor.
LEVEL 3
All Level 3 students will take a version of the HESI specialty exam at the start of each course. Students will take the HESI exams remotely or in a computer lab. It is highly recommended that the student reads all rationales once the HESI exam is completed. A score will be shown immediately after the exam is completed. This score will serve as a baseline and will help students determine areas of strength and areas that need improvement. Remediation packets will be created through HESI Assessment Next Generation. The purpose of remediation is to improve the student’s critical thinking, reasoning skills, and test taking strategies to achieve NCLEX-RN success. All essential packets will be read and case studies completed as part of remediation.
Elsevier’s Adaptive Quizzing for NCLEX-RN will also be utilized for remediation. All remediation must be documented, completed, and turned in to the instructor by the date indicated. A second HESI Specialty Exam given at the end of the semester which will count toward 10% of the course grade. The grade entered in the course gradebook will be the Conversion Score which is a weighted percentage score that is provided by HESI. If remediation is not completed, the student will be allowed to take the exam, but will have 10 points taken off of the associated exam.
A HESI Remediation Form will be made available to the student. After identifying areas for remediation and completing the form, a signed copy will be uploaded to Canvas and serve as a contract between the student and instructor.

The student will complete remediation in the areas for improvement based on the remediation policy
HESI Specialty Exams
The following courses in the program will have a HESI Specialty Exam given:
RNSG 2432: HESI Medical/Surgical Exam
Students will take the Medical/Surgical HESI Exam in RNSG 2432
The student will complete remediation in the areas for improvement based on the HESI Specialty
Exam score EAQs will be determined per course.
NCLEX-RN Live Review
During the final semester, students are required to attend an NCLEX-RN live review. This is considered part of the requirements for graduation and is part of the RNSG 2230 course Professional Nursing Review and License Preparation.
HESI Exit Exam (Only for RNSG 2230)
Students should aim to achieve a score of 900 or higher on each exam. Research demonstrates that students who score at 900 or above on the HESI Exit RN Exam are predicted to pass the NCLEX without additional preparation. The HESI Exit exam will be given in the RNSG 2230 course as part of the Compass online course. The content in the course itself will be considered the majority of the remediation for the HESI Exit exam. The student will also be required to complete remediation in HESI Next Generation (essential packets and case studies). A version will be given at the start of the semester, another version mid-semester, and yet another version will be given at the end of the semester after the 3-day Live Review. The HESI Exit Exam #2 will have the Conversion Score recorded in the gradebook for RNSG 2230.
For students who score ≥ 900 on the HESI Exit Exam #2:
The Conversion Score grade will be submitted in the gradebook and final grades will be submitted.
The student will complete remediation (case studies and essential packets).
The student should continue to practice NCLEX questions after graduation until the day prior to the scheduled NCLEX-RN.
For students who score <900 on the HESI Exit Exam #2:
The Conversion Score grade will be submitted in the gradebook and final grades will be submitted.
The student will complete remediation (case studies and essential packets, and study plan B).
The student will return to campus on a scheduled date during Finals Week and take the HESI Exit Exam #3.
For students who score <900 on the HESI Exit Exam #3:
The student will discuss areas of deficiency with an instructor 1:1 and complete the remediation based on the HESI Exit Exam #3 score.
The student should continue to practice NCLEX questions after graduation until the day prior to the scheduled NCLEX-RN.
An instructor will continue to check in with the student weekly until the date of the scheduled NCLEX-RN.
**The Affidavit of Graduation will be signed once all remediation and exams are completed.
