• 1. Dates of service in the Titan II?

  2. Which of the following describes your job: MCCC, DMCC, PTS, MFT, BMAT, Maint, Other?

  3. AFB stationed at?

  4. How many times did you go into the silo during your years in the Titan program?

  5. Did you work in a silo with known leaks in which the Titan II had to be replaced? Aerozine 50, UDMH/Hydrazine or Nitrogen Tetroxide leak?

  6. Did you ever witness the MSA sensing equipment being calibrated with known amounts of Hydrazine or Nitrogen Tetroxide?

  7. Have you had a head MRI or CT scan? Yes/No. Findings? Year of last scan?

  8. Have you had a chest MRI or CT scan? Yes/No. Findings? Year of last scan?

  9. Have you had an abdomen MRI or CT scan? Yes/No. Findings? Year of last scan?

  10. Have you had a pelvic MRI or CT scan? Yes/No. Findings? Year of last scan?

  11. Have you had unexplained muscle spasms/pain and where?

  12. Have you had alopecia (spots where hair does not grow) on the face or other parts of the body?

  14. Have you had foot pain problems?

  16. Have you had any other medical problems?   21. To verify you are a Titan II member please tell me how many thick steel blast lock doors are there in the blast lock area? Remember they were interlocked in pairs.
