The Positive Outcome Process are questions designed to support a person to evaluate their behaviour and the generate options for change.
Perceptions: describe the situation as it currently stands. What is happening NOW.
Wants: describe the realistic ideal. How you want it to be.
Evaluation: is what you are currently doing getting you want you want?
Options: What else could you do?
Plan: What are you going to do now?
Possible script:
Tell me your story/What is the situation?/What is happening for you?/What do you want to tell me?
What would you like to be happening?/What would you like to be doing?/If things were happening the way you want them, how would that look?/What would help you to feel good?/How would you like things to be?
What have you being doing so far to get...?/What have you tried?/What has worked?/What has not worked?/What other things have you tried?/Did it solve the problem?/Did things get better or worse when you did...?/Did you get what you wanted?/Did it make you feel better or worse?/
What else can you try?/What other solutions do you have?/Can you think of something else you can do to fix the problem?/Do you want me to help you to find a solution?
Tell me your plan./What are you going to go and do now?/What are you going to do if this happens again?
