MEDICATIONS AT SCHOOL

Students may possess over-the-counter pain relief medication in the original container as long as consent has been given in Skyward on the "Self-Administration of pain relief medication Authorization" form (completed during school registration/enrollment). The "Self-Administration of pain relief medication Authorization" form must be completed annually. This privilege may be rescinded if there is concern of medication misuse. 


The Fairmont Area School District encourages and allows students to self-carry Asthma Inhalers and Epinephrine Auto-Injectors when the appropriate Authorization Form is completed and on file in the Health Center. Please contact the school nurse for specific procedure information. The Authorization Forms must be completed and renewed annually. Please contact the school nurse to ensure a health plan is developed for your student.


Prescription medications must be administered by trained staff in the Health Center.  All prescription medications will require the following form to be completed by a parent/guardian and health care provider: "Fairmont Area Schools Consent Form for Administration of Medication During the School Day". The "Fairmont Area Schools Consent Form for Administration of Medication During the School Day" form must be completed/renewed annually. Prescription medication must be delivered to the school in the original pharmacy container (Including the appropriate label) by an adult. Please contact the school nurse for specific procedure information. 



ILLNESS AT SCHOOL

Students requesting to go home during the school day (due to feeling ill) must first be evaluated in the Health Center by the School Nurse or other Health Center Staff.
