Medical Education Outcomes Center (MEOC)
MEOC can be helpful with research/survey design questions in a consultative manner, with data acquisition from existing databases, and with data analysis.
It's always most helpful to residents if they have an identified faculty mentor who can guide/assist through the life of the resident project.
MEOC is most helpful if projects are focused on a medical education research question in some context.
The sooner that residents reach out, the better chance of success in terms of MEOC capacity to assist.

Typically, in a situation like this, I would recommend a single-sided non-scannable paper survey, that could be easily duplicated on any copy machine. Single-sided is better, because it is easier to use on a clipboard than something that is printed double-sided and has to be rearranged every time you turn the page.
Questionnaire design takes 30 - 60 hours, depending on the length of the survey, the number of questions, and the number of revisions needed. If you provide a draft of the survey, that generally reduces the cost. The OMS billing rate for this type of consulting is estimated to be $123.50/hour for the fiscal year that starts July 1, 2021.
OMS can also provide the services that will be needed when the surveys are completed, including coding/editing, transcription of open-ended responses, data entry with verification, and setting up an analysis-ready SPSS data file. The cost to provide these services would depend on the number of completed surveys, and the length of the survey (# of questions or # of pages). If you could tell me what to assume, I could give you an estimate for these services as well.
The cost would not vary between faculty and resident doctors. Our costs vary depending on if our client is an internal university client or external to the university.
For the scenario you described if our office is just advising on questionnaire design I would roughly estimate from around $100-$500. The range and costs of a project can really vary depending on the scope of the project. We have large-scale projects that our office consults on, creates the survey, administers the survey, conducts data analysis from A-Z, and some projects where our office only handles small pieces of the projects.
For any time/service that we would be working on we would have to charge for our time.
