Type of elective: Clinical
Objectives:
To integrate and act as both a teacher and learner in the context of the inpatient medical team, which includes pediatric faculty, advanced medical officers, interns, other internationals, and nursing personnel. Attempt to encourage participation and learning at all levels when appropriate.
Engage in bedside teaching, formal lectures, conferences and other programming, especially on pediatric topics.
To compare and contrast care between Selian and Arusha Lutheran Medical Center (ALMC) hospital wards, recognizing the differences and disparities between high and low-income Tanzanians, rural vs urban populations, and cultural groups.
To understand the complexities of decision making in low-resource settings, especially when disagreement about best practices between U.S. and Tanzanian medical practices arise, considering solutions that involve the input and expertise of practitioners from both backgrounds.
To be active in the practice of medicine in a resource-poor country you may be required to help/perform technical procedures, eg. start an IV, place a Foley catheter, perform a lumbar puncture, etc.
To participate in outpatient management of acute and chronic diseases.

Estimated Costs
Airfare - $1700 - 2200 to JRO airport, plus $60 for taxi to/from Arusha to airport.
Travel Insurance - through CISI (~$25 per month)
Travel Meds: Doxycycline - $20 (optional, depending on itinerary), Azithromycin - $30
Travel Vaccines - Covered with HP insurance
Medical License + government fees - $300
Ground Transport - variable, variable. Walking to ALMC is usually a nice option, but a taxi may be needed to Selian or farther around town (typically $4 - 8 one way, often split between residents). Dala dalas (public buses) are much cheaper, and easy to navigate.
Visa - $250 for business Visa
Phone/SIM cost - $45
Housing - $300-400 per month
Housekeeping: ~$20
Food - <$10 per day if always eating out, much less if cooking at home

Type of Experience: Clinical and research
Is a Temporary Medical License Required? Yes and will be arranged for you. Current cost is 300 USD + 100 USD fee of the hospital.
Can a VISA be acquired upon arrival? Yes it will be obtained on arrival at the airport. You will require a letter of authorization from Arusha which will be provided for you.
Primary Language Spoken: Swahili
Are language skills required in the primary spoken language: No, but would be good to have basic proficiency.
Housing: Currently 400 USD a month and provided. Of note, 40-50 USD cab ride from the airport to the house.
Dress code: Formal in the hospital. Ties not needed.
Travel Medicines: see CDC for Tanzania for up to date information, take all prescription medicines in the prescription bottle.
Required Vaccines: see CDC for Tanzania for up to date information, consider rabies vaccine.
History:
Primary hospital for most will be Selian which is a rural hospital serving the indigenous Maasai people. It has very basic resources and is a great place for learning. Has Medicine, Peds, OB, and Surgery along with an outpatient clinic. About 3 miles from the house and can get there by walking or bus. Usual hours are 7:30 a.m. to 4:00 p.m. Arusha Lutheran is in the city proper and about a mile away. Busy urban hospital with a lot more resources including an ICU and hemodialysis.
