State law requires that ALL STUDENTS have a completed health information form signed by a parent or guardian, and a medical form with proof of up-to-date immunizations signed by a doctor, on file in the school office. (These forms MUST be in English.) You may be excluded from classes until these requirements have been met.
If you have an accident or become ill at school, report immediately to Val in the main office. She will notify your parents and call for medical assistance if it is serious enough to warrant it.
Your parent/guardian must provide a written request for school personnel to hold any long-term medication (more than one dose) for you. Medications are kept locked in the main office. It is your responsibility to obtain your medication from office personnel; the staff will not seek you out.
If you have a condition requiring the use of an inhaler, parents must inform the office. If you are competent in the management of your respiratory condition, you may keep your inhaler with your belongings. It is strongly encouraged that you leave a back-up inhaler in the main office. Similarly, you should keep your EpiPen® with you if you have severe allergic reactions that warrant immediate intervention. If needed, you may keep your insulin in the refrigerator near the main office.
