Skin Lesion Form:

• • Pimples or spots that appear on body suspected as infections require treatment 72 hours before entering the mat.

  • Lesions include Ring Worm, Impetigo, Staph, Molluscum, Herpes are all treated differently so use physician treatment.

  • Lesion forms may be signed by doctors or Patient First centers to prove treatment/evaluations were started in time.

Medicines:

• Coaches will not provide medicines to athletes (including treatment for headaches). 

• Trainers should be aware of medicines required for athletes.
