Epimeria

The picture I based my design on made it difficult to tell what was going on with the legs of this thing, so I did a bit of reading about them. An epimeria is a type of amphipod meaning it has eight pairs of legs - one pair points toward it's mouth to help it eat, four pairs point outward, and three lower pairs point downward towards the tail. The body ended up stretching a little longer than I expected, so the tiny flaps in the bottom left corner of the crease pattern shown below became irrelevant - I ended up pleating the tail back into the body in order to shorten it.
