A fatal accident occurred at Douglas Station with No.3 Pender colliding with the buffer stops having departed Union Mills without a brakesman resulting in a lack of stopping power; the driver managed to jump clear of the train as it approached the platform but the fireman struck a canopy stanchion and was killed on impact. This resulted in an enquiry and ultimately saw the introduction of vacuum braking.
