TFI 3.6 RESOURCES: Student/Family/Community Involvement
Tier 3 team has district contact person(s) with access to external support agencies and resources for planning and implementing non-school-based interventions (e.g., intensive mental health) as needed.
Systems support the accurate and durable implementation of practices by staff, the efficient use of data for decision-making, and achievement of outcomes. Staff are trained to prevent many student behavior problems as well as to deal with disruptive behaviors in a proactive and positive manner. Systems provide the procedures and infrastructure to support and maintain new evidence-based practices.
TFI 3.6 Big Ideas
In element, TFI 3.6, the Tier 3 Consultant will work with a member of the SWSS Department - Tier 3 Point of Contact (PoC) to include district-level and community supports in each student's behavior support plan, when appropriate.
SWSS Department - Tier 3 Point of Contact (PoC)
The Student Wellness and Support Services Department is dedicated to supporting sites to build Tier 3 supports, including assisting schools in identifying internal supports and external agencies to support students with Tier 3 needs. The Mental Health Project Team has been assigned to ensure all schools feel equipped and supported in developing Tier 3.

In-District Supports
The HEART Team currently consists of 4 Licensed Marriage and Family Therapists and a team of Associates.
The HEART Team provides group counseling and individual therapy for general education students exhibiting internal/external behaviors stemming from symptoms of:
Depression,
Anxiety, and/or
Acute Trauma/Single Incident Trauma
Community Agencies
The Mental Health Project Team will also help connect families to community agencies for internal/external behaviors which stem from: sexual abuse, non-medicated ADHD, substance abuse, etc.

Loma Linda BMC specializes in supporting students with Major Depression, Bulimia Nervosa, and Substance Abuse/Chemical Dependency.

Victor Community Support Services (VCSS) specializes in supporting students receive a Mental Health Assessment, Psychological Assessment, Individual and Family Therapy, Medication Management/Support, Targeted Case Management, Rehabilitation Services, Crisis Intervention, and Therapeutic Behavioral Services.

How to Score 2 Points on TFI 3.6
To receive a 2 Point Score Teams must be able to say yes to the following set of questions.
When evaluating the 3 randomly selected behavior plans:
a) The Tier 3 Systems Planning Team has identified the SWSS Tier 3 Point of Contact (PoC) from SWSS,
b) Each randomly selected plan's Unique Student Support Team worked with the SWSS Tier 3 Point of Contact (PoC) to identify in-district and/or outside agency supports, and
c) The in-district/outside agency supports recommended by the SWSS Tier 3 Point of Contacted (PoC) are documented into each of the randomly selected plan.
