Self Placement Forms

Please click on the relevant button for the required form.

The forms will need to be completed in the following order:

1. Work Experience Signature Document (this requires your signature, your parents signature and the Employer/Organisation - This would have been handed out to you

2. When the signature form is complete, hand it into the lower sixth form office

3. Self Placement Form

4. Evaluation Form (Your work experience is officially complete when you complete and submit the evaluation)
