Home Education Program Information and Templates
All children aged six through 18 must comply with compulsory school attendance requirements. To meet these requirements, parents must ensure that their child between the ages of six and 18 is attending or participating in one of the following: a public elementary, middle, or high school; a public charter or cyber-charter school; a private licensed academic or private religious school; home tutoring by a certified teacher; or an approved program of home education.

Homeschool Student Registration Process

Registration Deadline
A notarized affidavit must be filed with the school district before you start your homeschool program and by August 1 of each year after that.
Beginning the Program - Registration Requirements
If your child is a current student in the district, a student withdrawal form must be completed before registering as a home education student. The form is obtained from the school your child currently attends. Failure to complete this process may result in truancy citations. Following withdrawal, a notarized affidavit with attachments must be filed with the school district before you start your home education program and then annually by August 1 thereafter.
If your child is transferring into the district, you should first withdraw your child from the current district. Request that a letter of transfer be sent to Eastern York School District. You may then submit the affidavit with attachments to Eastern York.

Affidavit listing the student(s) in the Home Education program, signed and properly notarized. The following sample affidavit / unsworn declaration documents are available to download to begin or continue your child’s home education program:
Proof of Physical Examination / Immunizations (to be completed by a physician) as appropriate for each homeschooled student. (click HERE for the Pennsylvania Department of Education’s requirements) or Physical Exemption Form.
Evidence (records or a letter from the physician) that the child or children have been immunized as required by School Code Sec. 1303(a) unless a physician certifies the child has a medical contraindication to immunization and
Evidence (records or a letter from the physician) that the child or children have received medical services required for students of the child’s or children’s age or grade level in Article XIV, or religious exemption for these two requirements.
An outline of objectives for each student, by subject area, for the upcoming school year.
Students with Disabilities: If the student has been identified as a child with a disability pursuant to the Individuals with Disabilities Education Act (IDEA), written notice of approval of the home education program by a licensed clinical or certified psychologist or a PA-certified special education teacher must be submitted along with the affidavit.
Additional Opportunities for Homeschool Students
Home-educated students may participate in any co-curricular activity that merges extracurricular activities with a required academic course, academic courses (equaling up to at least one-quarter of the school day for full-time students), and career and technical education (CTE) programs. Once this initial documentation is received, reviewed, and deemed satisfactory, the Supervisor (parent/guardian) of the Home Education Program will receive a letter from EYSD stating that all requirements for the coming school year have been met and that you may begin your home education program. The letter will also outline the items due to the Office of the Director of Teaching and Learning no later than June 30th of each year. If your child is currently enrolled and attending EYSD, you will need to visit the building of attendance and officially withdraw your child upon receipt of the letter.
Upon satisfactory receipt of the above items, the Supervisor will receive a letter from EYSD stating that all requirements for the coming school year have been met and that you may begin your home education program for the current school year.

End-of-Year Documentation Process
Throughout the school year, the supervisor must maintain a portfolio of student work. The portfolio must be evaluated at the end of each year by:
Licensed clinical psychologist;
School psychologist;
Pennsylvania certified teacher with two (2) years grading experience at the level they evaluate (K-6 and/or 7-12);
Nonpublic school teacher or administrator with two (2) years PA teaching experience within the last ten (10) years at the level they evaluate (K-6 and/or 7-12);
Persons with other qualifications, with the prior consent of the Superintendent of the District of residence (This is completely at the discretion of the Superintendent. Some examples of requests include college professors, teachers certified in other states, experienced homeschool parents/guardians, professional curriculum providers, and teachers in "umbrella schools.").
Achievement tests aid in determining whether or not an appropriate education is occurring. The law requires that home education students take the statewide tests or other authorized tests in grades 3, 5, and 8.
The evaluation is arranged by the supervisor. If not previously provided, evidence of the evaluator’s certification must be submitted along with the evaluation.
At the conclusion of each educational year, the home education supervisor must submit a written evaluation of the home education program to the Superintendent of Schools in the student’s district of residence no later than June 30th for the closing school year.
