IMPORTANT Update and survey

Dear North Country NYS Master Teachers—

I hope that your fall semester is wrapping up well for you, your family and your students.

I write with the link for the mid-year program survey. Please complete this survey before or by Friday, December 30. The survey will take approximately 30 minutes.

In my November STEMreach issue, I recommended reflecting on your professional development activities this fall in advance of the survey. I look forward to learning about your experience as part of my commitment to continuous review and development of the NYSMTP.

When prompted to sign in, your email address is your username.

Click "forgot password" and follow the email instructions to create a password.

This allows you to start, save your answers and return to the survey—should you choose not to complete the survey in one sitting.

Important:

  1. We strongly discourage using a wifi connection to do the survey. If the wifi connection is disrupted, your answers will not be saved because you will be automatically logged out of the online survey program, without any message to alert you.
  2. Be sure that your computer is not blocking pop-up messages.
  3. After clicking "submit", you will see one of two messages:
    1. A thank-you-for-completing-the-mid-year-survey message.
    2. OR
    3. A large message in a red box at the top of the form that indicates incomplete or error items.
  4. Another way to verify that your survey was complete and successfully submitted, is to "View your stored results" after you are logged in. If you do not see a green check , your survey was not completed.
