Our Ob/Gyn colleagues are located across the street on the East Campus building, at the furthest point of campus from the ED. It typically takes 15-20 minutes to walk to the ED.
We do have fetal monitoring and a neonatal resuscitation bed in the ED. We also have the capability to perform stat c-sections in the West Campus OR if Ob calls for it.
For a small percentage of patients, it is okay to ask a clinical question over the phone to our ob/gyn colleagues without requiring them to come across the street for a full examination. However, if you do request an in-person evaluation from ob/gyn, they are required to come without exceptions.
Ob STAT emergencies, such as imminent deliveries, require somebody to use the red "Batphone" located in the Red Zone that connects immediately with the Labor & Delivery floor. Your conversation will trigger the on-call L&D and NICU team to jump into an ambulance and race across the street. This will typically take 5-10 minutes so in the meantime, you should be ready to deliver the baby and stabilize your patient(s).
Pregnant patients >=22 weeks with an Ob-related issue who are stable after a medical screening exam should be transported to L&D for Ob management after consulting them on the dashboard. See the attached protocol below.
Patients who are unstable or have major trauma should be managed by the ED with immediate Ob consultation
Patients who require West Campus diagnostic resources may stay on the West Campus only after consultation with Ob.
Occasionally, the attending will be called overhead to Triage to assess a pregnant patient in labor. You should gather a history and perform a limited examination that sufficiently rules out imminent delivery. The triage nurse will then book an ambulance to transport the patient immediately to L&D. You are required to write a note.
