Charting

Expectations:

• Admitted patient: note should be completed at time of admission and no later than the end of the current shift 

• Transferred patient: note must be completed prior to patient being transferred 

• Discharged patient: note should be completed within 48 hours of discharge 

• Insurance companies have recently threatened to withhold payment for any charts not completed within 7 days of patient visit. Therefore, chart completion is a high priority for the department

Expect Gary Shillin to send periodic emails reminding you about outstanding charts. Significant cases of delinquent charts will be referred to ED leadership.
