Discharge

Click "D/C Plan" and fill out discharge paperwork through the dashboard including:

• Diagnosis
• Appropriate prescriptions
• Appropriate follow-up (with PCP and/or specialists)
• Warning signs & home treatment.

At the bottom section, write a brief description of the visit (or use the “import MD comments” option) to communicate visit details with PCP. Please note these comments will become part of the OMR chart but will not be visible to patient on discharge sheets. Residents will complete discharge paperwork for their patients.
