The Incident/Accident form is now combined and you will find it below in the files.
The witness statement is also added below which will replace the old Incident Report. Please have all reports where someone would fill out a statement use this form.
Incident logs should be kept for all major incidents (Level 1 incidents) that involve multiple steps or multiple people performing different tasks that should be noted.
In all cases, if unsure - document it all.
