Driver approval information

Any Y staff who will be driving a Y vehicle must have a completed check IN ADDITION to any Y staff using their own vehicle for travel and submitting for mileage reimbursement.

• Please complete this simple form that will go to People and Culture approval.  

• You will also need to give them a copy of the Fair Credit Act document.
