Please use this site to submit any maintenance requests for facilities at EWH. Instructions to submit a request can be found here.

Student Travel Form

Please use this form any time you'll be staying overnight away from the WEPO complex. 

Overnight Guest Form

Please use this form to request permission to welcome an overnight guest at EWH.

Student Subsidy Form

Please use this form to submit receipts for your subsidies and bus passes.

EWH Arrival Form- Room Damage Declaration

Please use this form to declare any damage to your room within 24 hours of your arrival to EWH.
