Please read the Youth Formation Handbook and all registration articles. These provide some important information.

• Registration is for ages 14-17.  Those 18 (Or soon to be 18) and older (Will be 18 by December 1st of the enrollment year) must enroll in adult confirmation.

• If this is the candidate's second (2nd) year and they are going to be 18 before December 30th, but are still enrolled in High School please make sure the proper areas in the enrollment form are checked.

•  If this is the candidate's first (1st) year and they are going to be 18  before December 30th, but are still enrolled in High School, they may be deferred to an adult program.

• Required for Registration

  1. Registration packages may be picked up at the parish office:

     • Please read the information , terms and agreement document.

     • Fill out the Enrollment Form.

     • Parents are to Initial, sign and date the Agreement Form.

     • Fill in the Emergency Card

     • Sponsor form

  2. At the time of Registration you must provide:

     • Full payment (Checks or cash Only!)

     • Birth Certificate, 

     • Baptism Certificate **

     • First Holy Communion Certificate.*

     • A completed enrollment package.

** If candidate is entering the confirmatio program for all three sacraments of iniitioan (Baptism, Confirmation & Holy Communion); then note this in the proper area of the form
