Please read the Adult Formation Handout. This provides some inportant information.

• Registration is for those 18 (Or soon to be 18) and older (Will be 18 by December 1st of the enrollment year)

• Required for Registration

  1. Registration packages may be picked up at the parish office:

     • please fill out the Enrollment Form.

     • Initial, sign and date the Agreement Form.

  2. At the time of Registration you must provide:

     •  Birth Certificate, 

     • Baptism Certificate 

     • First Holy Communion Certificate.

  3. If you are married, please bring a copy of your marriage certificate (Whether it was a Catholic Matrimony, of another faith or a Civil Marriage)

Adult Confirmation and OCIA:

Cost per person:               $ 200.oo  per Adult

Continuation:                    $  75.oo  per Adult

Pre-approved Matrimonial preparation:  $  75.oo  per Adult
            (Late fees do not apply for this)

Other Fees: Late Registration Fees:*   $ 30 / Student  / Non-Parishioners add: $20 per student.
