Please read the Adult Formation Handout. This provides some inportant information.

• Registration is for those 18 (Or soon to be 18) and older

• Required for OCIA Registration

  • Registration packages may be picked up at the parish office:

    • please fill out the Enrollment Form.

    • Initial, sign and date the Agreement Form.

  • At the time of Registration you must provide a Birth Certificate.

  • If you are married, please bring a copy of your marriage certificate (Whether it was a Catholic Matrimony, of another faith or a Civil Marriage)
