Abstract

Salivary cysts in parotid gland of nonneoplastic makeup are quite rare and represent approximately 2-5% of all salivary gland lesions. Salivary duct cysts are also known as sialocyst is a true cyst as it has an epithelial lining in the inner walls. Generally observed in the minor salivary glands, it rarely affects the parotid and are mostly seen in between 30 and 40 years of age. The current case is a rare case report as it has been seen in a female in her 50s and that too in the parotid gland.

A Novel Modified Post-Ramal Sling Eversion Approach For The Treatment Of Subcondylar Fractures

Abstract

Objectives: The aim of this prospective study to assess the efficacy of a modified postramal sling eversion approach used for the treatment of subcondylar fractures. Because this approach allows direct access to the posterior surface of the ramus of the mandible and it provides a short distance to the condyle and sigmoid notch. Materials and Methods: Subcondylar/ high ramus mandible fractures were operated via the postramal sling eversion approach using delta plate fixation in 10 patients. The patients were evaluated for hematoma, infection, salivary fistula, facial nerve damage, occlusion, fracture site stability, chronic pain in the fracture site, hypoesthesia in the postoperative period. Results: The modified postramal sling eversion approach was found to be expeditious inadequately exposing the fracture site and enabling ORIF. In all cases, a good anatomical reduction was achieved with stable occlusion and without any signs of facial nerve impairment, however, the serous discharge was noted in one case. Conclusion: The modified postramal or ramal sling eversion approach provides minimal invasion and adequate exposure of the subcondylar region and it has minimal complications. So it should be considered as a commendable alternative in the management of subcondylar fractures. Prevalence and Pattern of Third Molar Impaction: A Retrospective Study of Orthopantomograms

Abstract

Objectives: The aim of this retrospective study was to investigate the prevalence and pattern of third molar impaction in patients between 19‒26 years old attending a singleinstitute.

Methods: The study reviewed 1,000 orthopantomograms (OPGs) of patients attending our institute.Patients were evaluated to determine the prevalence of third molar impaction, angulation, level of impaction and associated pathological conditions. Results: The most common angulationof impaction in the mandible was the mesioangular (38.60%) and the most common level of impaction in the mandiblewas level A. There was nosignificant difference in the frequency of impaction between the right and left sides of both jaws. Pathologicalconditions associated with impacted lower third molars were found in 7.1% and21.4% of impacted lower third molars were associated with dentalcaries.

Conclusion: Our study highlights mesioangular impaction as the most frequently encountered angulation in impacted teeth.

Abstract

TITLE The Impact of Diagnosis, Extent and Treatment modality of mandibular benign lesions on the supposedly repositioning of the Inferior Alveolar Canal – A retrospective observational study OBJECTIVES To identify the displacement and relocation in the position of the Inferior Alveolar Neurovascular Bundle after removal of benign lesions from the posterior mandible. MATERIAL AND METHODS 31 patients having a benign pathology in the posterior mandible underwent preoperative computed tomography scans to identify its relation with the Inferior Alveolar Nerve. All patients were operated under General Anesthesia and were divided into 3 groups based on the presence or absence of encroachment of the nerve, histologic diagnosis and mode of treatment and panoramic radiographs were taken to measure the change in displacement and subsequent repositioning of the nerve for a minimum follow-up period of 12 months. RESULTS The mandibular canal relocated to a mean (SD) distance of 2.15 (1.41) mm for patients in whom a benign tumor was excised (p <0.05) and a mean (SD) upward shift of 1.79(1.31) mm of the canal was seen in patients with nerve encroachment. (p<0.05).In patients where cystic enucleation and peripheral osteotomy was followed by chemical cauterization with carnoy’s solution or 5- flourouracil application, a mean difference of 1.58 mm was seen in the preoperative position of the nerve. CONCLUSION A superior remodelling of the canal was noticed in cases where there was absence of a superior bony barrier in the form of an intact cortex of Inferior Alveolar Nerve or encroachment of the lesion was seen in the nerve. It was concluded that the selection of a particular treatment modality has no impact on the subsequent relocation of the mandibular canal and grafting should be accomplished only in cases wherein future rehabilitation is desirable or anatomic requirements indicate.
