Before starting the process, we encourage you to take a moment to review NKY Health's Guide to Personnel Policies on the ADAAA and light duty. This will help you understand the requirements and ensure your situation aligns with them. Once you've done that, the process is simple and designed to get you the support you need as quickly as possible.
Download the Form: Start by getting the "Request for Medical Accommodation" form (below). Fill out the employee sections.
Talk to Your Health Care Provider: Take the form to your health care provider. Give them a copy of your current job description so they can understand your role. They will fill out the medical sections of the form. (Alternatively, the health care provider may provide the same information in a different format as long as all the information is included and it is in writing.)
Review and Submit: Once your health care provider has completed the form, take a moment to review it to ensure everything is accurate and matches your understanding of your needs.
Send it to HR: When you're ready, submit the form to the Head of HR. For detailed instructions on how to securely submit forms, refer to "How to Submit Forms/Documents to HR".
