We understand that talking about life insurance can feel a little heavy, but it's one of those important benefits that can provide peace of mind for you and your loved ones. We've worked to make our plan as straightforward as possible, and we're here to help you navigate it.
At NKY Health, we're happy to provide all full-time employees with a free Group Life Insurance policy. This coverage, which includes $20,000 of accidental death and dismemberment coverage, is on us! This coverage, provided through Minnesota Life via OCHS, becomes effective on the first day of the month after your first full month of employment. For example, if your start date is May 10, your benefits begin on July 1. To ensure you're covered, it's essential that you actively enroll in the plan.

Additional Coverage for You and Your Family
Beyond the base coverage, you have the option to purchase supplemental term life insurance for yourself and your eligible dependents whenever you want. You can enroll at any time; however, be aware of the "enrollment period."
For your first 31 days, you have a special opportunity! You can enroll for a certain amount of additional coverage without any medical exams or health questions. We call this 'Guaranteed Issue.' Here are those special amounts:
For you: up to $150,000
For your spouse: up to $25,000
For your children: all coverage amounts are guaranteed!
After these first 31 days, or if you want more coverage than the amounts listed above, you'll need to submit some health information for approval. If you opt for additional coverage, the cost will be split evenly across your first two paychecks each month, with deductions starting when your coverage begins.
Please keep in mind:
If your additional coverage is based on a multiple of your annual salary, the amount of coverage and the premium will be adjusted in January following any salary change.
Premiums for additional coverage may change as you get older, based on the age bands described in the plan's brochure.

Welcome! Enrolling in your group life benefits is a key step, and we're here to help make it as smooth as possible. Follow these three simple steps to elect and enroll in your coverage.
Step 1: Fill Out Both Forms
Group Life Enrollment Form: Download and fill out this form.
Section A: This section is for your coverage. Leave the numbers in Column 1 as they are. In Column 2, enter the additional coverage you want. If you only want the basic plan, write '0'. Column 3 is the total of Column 1 and Column 2. Be sure to enter your benefits effective date in all four required fields.
Section B (Spouse): Complete this if you are electing coverage for your spouse.
Section C (Children): Complete this if you are electing coverage for your children.
Group Life Beneficiary Designation Form: Download and fill out this form.
Important: The percentages you assign to your beneficiaries must add up to 100%.
Step 2: Sign and Send Both Forms to HR
Once both forms are complete, sign them with a blue or black pen. You must submit both forms together.
Email: Scan both signed forms and email them to HR@nkyhealth.org.
We will review your forms and fax them to our group life representative for processing.
Step 3: Keep Your Copies
After you have submitted your forms, be sure to keep the original signed copies with your important papers for your records.
Extra Information
Evidence of Insurability (EOI): If you're electing additional coverage after your first 31 days of employment or a coverage amount above the guaranteed issue limit, you may need to complete a "Evidence of Insurability" form. This form must be sent directly to Minnesota Life Insurance, not to HR.

Changing your group life coverage is a simple process, and we're here to help you through it. You can decrease or stop your coverage at any time. You can also increase your coverage at any time, subject to approval by underwriting.
To get started, just follow these easy steps:
Download and fill out the Group Life Enrollment form. You can enter the information right on your computer.
Complete the "Voluntary Life" section:
In Column 1, enter your current amount of voluntary coverage.
In Column 2, enter the amount you want to add or subtract.
Column 3 should be the new total of Column 1 and Column 2.
Enter the first day of a future month as the effective date.
Print and sign the form using a blue or black pen.
Submit the form to HR.
Keep the original signed form with your important papers.
A quick note: If you're adding coverage after 31 days of your hire date or electing an amount above a certain limit, you may need to complete an Evidence of Insurability form. This form is submitted directly to Minnesota Life Insurance, not to HR.
The employee is automatically the beneficiary for any insured spouse or dependents.
