About NKY Health's Group Life Insurance

NKY Health contracts with Minnesota Life through OCHS to provide $20,000 of accidental death and dismemberment coverage at no cost to eligible full-time employees.  (This is the same amount of coverage that is provided to "state" employees but the cost is less.)

Benefits become effective on the first day of the month after the first full month following the date an employee is hired into an eligible full-time position. (For example, benefits for an employee hired on May 10 begin on July 1.) Employees must actively enroll/elect benefits to be covered.

Employees may enroll in/elect additional coverage for themselves or their dependents anytime but it will only be guaranteed issue up to a certain amount within 30 days from their hire/eligibility date. (Guaranteed issue means coverage is approved without underwriters requiring additional information or a medical exam.) 

Should an employee opt for additional coverage for herself/himself or a dependent beyond the $20,000 provided by NKY Health, payroll deductions for the coverage are split evenly between the first two payrolls of each month and will start in the month coverage begins.

How to Elect/Enroll In Benefits for the First Time

Unless otherwise instructed by the HR Administrator, follow these steps to elect/enroll in benefits as a new employee or as an existing employee who is newly eligible for benefits:

(Employees who elect additional coverage after 30 days or elect coverage above a certain amount must complete an "Group Life Evidence of Insurability" form and submit it directly to Minnesota Life Insurance.  (See "Contact Information" below.)  Do not submit this form to HR.)

How to Change Your Group Life Level of Coverage

Employees can decrease or stop their coverage anytime and increase their level of coverage anytime (subject to approval by underwriting).  To initiate the process of changing coverage, follow these steps:

How to Change Your Group Life Beneficiary

The employee is automatically the beneficiary for any insured spouse or dependents.

Employees can change the beneficiary for their own insurance any time.  Follow these steps to change the beneficiary for your own insurance:

Legacy Planning Services

Minnesota Life contracts with Securian so you can access a variety of online information/resources, including end-of-life and funeral planning, final arrangements, important directives and survivor assistance. After a claim is started, these additional services are available to beneficiaries by phone.
