About NKY Health's Group Life Insurance
NKY Health contracts with Minnesota Life through OCHS to provide $20,000 of accidental death and dismemberment coverage at no cost to eligible full-time employees. (This is the same amount of coverage that is provided to "state" employees but the cost is less.)
Benefits become effective on the first day of the month after the first full month following the date an employee is hired into an eligible full-time position. (For example, benefits for an employee hired on May 10 begin on July 1.) Employees must actively enroll/elect benefits to be covered.
Employees may enroll in/elect additional coverage for themselves or their dependents anytime but it will only be guaranteed issue up to a certain amount within 30 days from their hire/eligibility date. (Guaranteed issue means coverage is approved without underwriters requiring additional information or a medical exam.)
Should an employee opt for additional coverage for herself/himself or a dependent beyond the $20,000 provided by NKY Health, payroll deductions for the coverage are split evenly between the first two payrolls of each month and will start in the month coverage begins.
Be aware that:
For coverage that are multipliers of annual salary, the amount of coverage and associated premiums will change the first January after a salary change.
Premiums for coverage elected beyond the $20,000 that NKY Health provides will change according to the age banding described in the brochure.
Optional benefits are also available through NKY Health's voluntary personal insurance provider.

How to Elect/Enroll In Benefits for the First Time
Unless otherwise instructed by the HR Administrator, follow these steps to elect/enroll in benefits as a new employee or as an existing employee who is newly eligible for benefits:
Download and complete the "Group Life Enrollment" form by entering the information electronically in the fields.
Complete the fields in the Voluntary Life section as follows:
In Column 1 leave the 0.00s.
In Column 2 enter the additional amount of coverage you want to purchase; if you do not want to purchase additional coverage, enter 0's in the fields.
In Column 3, enter the total of Column 1 and Column 2.
Enter the effective date four times (once near the top and three times in the columns on the right).
Print the form and sign it using blue or black ink.
Submit the form to the HR Administrator. (See "How to Submit Forms/Documents to HR" for detailed instructions about how to submit the form.)
Download and complete the "Group Life Beneficiary Designation" form by electronically entering the information in the fields using these tips:
The %ages for beneficiaries must total 100%.
The first page contains helpful information and must be included with the second page when you submit the form to Securian.
Print the form and sign it using blue or black ink.
Submit the form to Securian as per the instructions at the top of the form.
Keep the originals with your important papers.
(Employees who elect additional coverage after 30 days or elect coverage above a certain amount must complete an "Group Life Evidence of Insurability" form and submit it directly to Minnesota Life Insurance. (See "Contact Information" below.) Do not submit this form to HR.)
How to Change Your Group Life Level of Coverage
Employees can decrease or stop their coverage anytime and increase their level of coverage anytime (subject to approval by underwriting). To initiate the process of changing coverage, follow these steps:
Download and complete the "Group Life Enrollment" form by entering the information electronically in the fields.
Complete the fields in the Voluntary Life section as follows:
In Column 1 enter the amount of voluntary coverage you are currently purchasing.
In Column 2 enter the amount you want to add or subtract.
In Column 3 enter the total of Column 1 and Column 2.
Enter the first day of a future month as the effective date .
Print the form, sign it using blue or black in, scan it to your e-mail as a .PDF, and forward it to HR.
Submit the form to the HR Administrator. (See "How to Submit Forms/Documents to HR" for detailed instructions about how to submit the form.)
Keep the original with your important papers.
(Employees who elect additional coverage after 30 days past their hire date or elect coverage above a certain amount must complete an "Evidence of Insurability" form and submit it directly to Minnesota Life Insurance. (See "Contact Information" below.) Do not submit this form to HR.)

How to Change Your Group Life Beneficiary
The employee is automatically the beneficiary for any insured spouse or dependents.
Employees can change the beneficiary for their own insurance any time. Follow these steps to change the beneficiary for your own insurance:

Minnesota Life contracts with Securian to provide independent, objective and free financial coaching program for beneficiaries receiving $25,000 or more when a life insurance claim is paid. The benefits include:
Dedicated financial coaching: Available monthly via phone for help with financial decisions during first 6 months after claim is paid. Includes coaching the following 6 months, as needed.
Access to PwC Envision™: 12 months of mobile-enabled web application with budgeting, planning tools and content.
Financial fitness assessment: Personalized wellness report outlines key action items to discuss with a financial coach.
Survivor guide workbooks: Help make financial and legal decisions less overwhelming.

Minnesota Life contracts with Securian so you can access a variety of online information/resources, including end-of-life and funeral planning, final arrangements, important directives and survivor assistance. After a claim is started, these additional services are available to beneficiaries by phone.
Funeral concierge: Allows for coverage verification and direct payment to a funeral home so services can be provided before insurance payment is made.
Express Assignment™: Same-day funeral home assignment service reduces concern about paying funeral expenses by working with the funeral home or lending agency.

Minnesota Life contracts with Securian so you can access professional services for a variety of needs - from legal matters and financial situations to coping with loss - through comprehensive web and mobile resources, as well as consultations.
