How to Identify Yourself as an Individual Who Has a Disability
NKY Health maintains information about each employee's race and ethnicity in it's Human Resources Information System (HRIS).
To identify yourself as an individual who has a disability, please log into ADP's WFN (Workforce Now) and check the appropriate box related to personal information.

How to Request a Job Accommodation or a Light Duty Assignment
To request a job accommodation or a light duty assignment, please read NKY Health's policies related to the ADAAA and light duty to ensure that your situation meets the requirements. If it does, then please follow these steps:
Download the "Request for Medical Accommodation" form and complete the employee parts. (This form is designed to expedite your request.)
Take the form to your health care provider with a copy of your current job description and ask them to complete it for you. (Alternatively, the health care provider may provide the same information in a different format as long as all the information is included and it is in writing.)
Check the completed form/documentation to ensure the information is consistent with your understanding of your needs. If it is not, work with your health care provider to correct this.
When the form is complete and accurate, submit it to the HR Administrator. (See "How to Submit Forms/Documents to HR" for detailed instructions about how to submit the form and any supporting documentation.)
