Voluntary Personal Insurance

NKY Health contracts with Colonial Life Insurance to offer optional insurance that is completely voluntary, meaning that you pay 100% of the premium and discounted group rates. Some popular packages include Cancer, Hospitalization, Life and Disability insurance.  Any NKY Health employee is eligible to take advantage of group rates for voluntary personal insurance through Colonial Life.

(This is separate from the Group Life and Accidental Death and Dismemberment Insurance NKY Health provides at no cost to full-time NKY Health employees who enroll. Eligible employees can also elect additional benefits through this plan.)

A Short Term Disability package is offered as a guaranteed issue package to employees who enroll within 30 days after their initial hire date. (Guaranteed issue means coverage is approved without underwriters requiring additional information or a medical exam.)  After the 30 days, Colonial Life will only issue the coverage if their underwriters approve.  All new employees must read and submit the "Voluntary Personal Insurance" form found below to acknowledge that they understand the terms of this agreement with Colonial Life Insurance.

Otherwise, employees may elect, change, and end insurance at any time according to the provisions of their individual contract with Colonial Life. NKY Health assumes no liability for the contract provisions.

Note that optional accidental death and dismemberment coverage is also available through NKY Health's Group Life Insurance provider.
