At NKY Health, we're always looking for ways to make things easier and more efficient for everyone. To help us maintain accurate records and reduce paper waste, we've put together a simple guide for submitting your forms and documents to HR. Thank you for your support in these efforts!
Before you send us any paperwork, please take a moment to double-check that your documents are:
Legible: Is the text clear and easy to read?
Complete: Have you filled in all the necessary sections?
Accurate: Is the information correct and consistent? This is especially important for details provided by a third party.
Signed: Have you signed in all the required spots?
The easiest and most reliable way to send forms and documents is to scan them into a PDF and email them to us (hr@nkyhealth.org) from your @nkyhealth.org Gmail account. This creates a secure, digital record for both you and HR.
Easy Scanning Options:
NKY Health Scanners: We have scanners available at every one of our sites for your convenience.
Smartphone Apps: There are many free apps for your phone that can easily create high-quality PDF files from photos.
A Quick Guide to Submitting Scanned Documents:
Create a PDF file for each document. For forms with multiple pages, including double-sided ones, please scan them all into a single PDF.
Review your PDF. Make sure all pages are included, oriented correctly (right-side up), and the text is clear.
Email your PDF(s) to hr@nkyhealth.org. Please use a clear subject line, like "HR Form: Leave Request" or "Benefits Enrollment," so we can process it quickly.
Wait for our confirmation. We'll send you a reply to let you know we've received your email.
No worries at all! If a scanner isn't an option for you, you can also fax your forms.
Our Secure Fax Line: Please fax your documents to 859-363-2101. This is a secure line that only our HR team can access, ensuring your information stays private.
Just a friendly reminder: When faxing, please double-check that all pages are included and the text is clear and readable. For your privacy, please do not fax to NKY Health's general fax number or directly to your worksite.
We understand that sometimes technology just isn't an option. If you are off work and unable to use a scanner or fax machine, we still want to make sure you can get your documents to us.
Drop-off: You can have someone drop off the paper forms for you at the District Office.
