Signed: Have you signed in all the required spots?
The most reliable, efficient, and secure way to send your documents is by emailing a PDF from your @nkyhealth.org email account to hr@nkyhealth.org. This creates an immediate digital record for both you and HR.

Smartphone Apps: Many free apps can quickly create high-quality PDF files from photos using your personal device.
Quick Guide to Emailing Documents
Create a Single PDF: For multi-page documents, including double-sided forms, please scan them all into one single PDF file.
Review the PDF: Confirm that all pages are included, oriented correctly (right-side up), and the text is clear.
Email to HR: Send the PDF(s) to hr@nkyhealth.org. Please use a clear subject line, such as: Benefits Enrollment or FMLA Request.
Wait for Confirmation. We will send you a reply to confirm that your email and documents have been successfully received.
If a scanner is not available, you are welcome to fax your documents.
Secure Fax Line: Please fax your documents to 859-363-2101. This is a secure line accessed only by the HR team, ensuring your information stays private.
Important Reminder: Please do not fax to NKY Health's general fax number or directly to your worksite. Always double-check that all pages are included and the text is readable before sending.
