How to Submit Forms / Documents to HR

Before Submitting Forms / Documents

Before submitting any forms to the HR Administrator please review them to ensure:

Preferred Method of Submitting Forms / Documents

The preferred method of submitting any forms and supporting documents is by scanning them to a .PDF and forwarding them from your fname.lname@nkyhealth.org Gmail account.  (When you do this, both you and HR have a record of what was sent and when.)

NKY Health has scanners at every site and most smart phones allow you to download a free app that you can use to produce .PDF files.

Please follow these steps to submit forms/documents by scanning:

HR will reply to your email to confirm receipt.

Alternate Method of Submitting Forms / Documents

If you do not have access to a scanner, you may fax the forms to 859-363-2101. This is a secure fax line that only HR uses.

Please be sure that all pages are in the file, right-side-up, and of sufficient quality to be legible.

Please DO NOT fax to NKY Health's general fax number or to your worksite.

As a Last Resort

As a last resort (when you are off work and do not have access to a scanner or fax machine AND cannot create a .PDF from your phone) you may have someone drop off the paper forms for you or you may send them via U.S. Mail to:
