NKY Health provides Workers' Compensation insurance to cover you in the event of a work-related injury or illness. Your safety and well-being are our top priority.

If you experience an injury or illness during work time, you must report it immediately to your supervisor.
Reporting and Forms: Notify your supervisor immediately of the injury or illness and complete the required Employee Incident Report form.
Coordination of Care: After your supervisor completes the initial reports, you must coordinate any required medical attention (beyond first aid). This is done by seeing an approved "Gatekeeper" provider or by calling the claims service directly for guidance: 1-866-367-5226.

Please ensure you carry your blue-and-white card at all times. You must present it to the provider when seeking care for a work-related injury or illness. If your card is lost, please contact Human Resources (HR) for a replacement.

As a reminder, all employees are required to read and acknowledge the Workers' Compensation policy upon hire and annually thereafter. Please ensure you complete and submit this form as directed.
