Ressun lukio brag sheet for recommendation 

You can use this template when providing your recommender background information about you for the recommendation letter. You don't need to use all question samples. When you send the brag sheet, please write both the question and your answer.

1. Your name and contact information 

2. The University and program you plan to apply 

3. List the course(s) you took with this teacher 

4. Your latest predicted grades (When they were published and each subject+grade) 

5. Your standardized test scores (SAT, ACT etc.) 

6. Activities you have participated in and when 

7. Academic clubs you have attended and what has been your role in them 

8. Sports teams you have attended and what has been your role in them 

9. Extracurricular clubs you have attended and what has been your role in them 

10. Out-of-school clubs you have attended and what has been your role in them 

11. Leadership roles you have held 

12. Work experience and when 

13. Any competitions you have participated in and any awards you have won (academic, athletic, etc.) 

14. Describe a project (EE/IA or other) you worked on in the class that you are proud of

15. Your goals for the future as it relates to your college applications 

16. List three adjectives you would use to describe yourself

17. Attribute describing you and how it showcases
