Surveys and QR Codes
Surveys
Google Forms Allows you to create and administer a survey quickly and easily. You can build a survey from a variety of question types (checkbox answers, matrix answers, scale answers, text answers). Questions may be easily re-organized, and you can create multiple "pages" based on responses.
You can then share or publish the form online for people to take. All responses are automatically tallied and presented in a graphic format, or may be downloaded in Microsoft Excel format.
- Sign in to Google using your existing gmail or Google ID (or create one for free),

QR codes - a printable, scannable hyperlink
QR codes are great to get people directly to a webpage, no typing required! (SmartPhone, Tablet or iPad required)
"Quick Response" codes are similar to traditional barcodes, but can store more information.
To create:
1. Find a website (or Google form) you want to link to and copy the address
2. Take the address to a QR code generator like
- Goo.gl (Google URL shortener) Click "details"
- Kaywa (choose "standard" instead of "dynamic")
- Deliver QR
- QR Stuff
3. Save / download the QR code image
4. Put the image in Microsoft Word, Publisher, etc. to print and hand out! (including a heading or brief instructions would be nice as well)
To use:
1. Open any barcode scanning app on a SmartPhone, Tablet or iPad with a camera (popular ones include "RedLaser," "ScanLife," "ScanIt," or "QR Reader"
2. Scan the code
3. Take the survey
