Parent Forms

Please return any of these forms (if needed) to the front office.

It is very important that we have up-to-date information.

Please notify the front office of any changes.

Alternate Pick Up Form ( please complete this form if your student(s) will be picked up by anyone other than a parent/guardian)

Medication Form - ANY medication (prescribed or over-the-counter) requires medication form signed by your child's physician. This completed form must be kept in the front office along with any medications.

Walk-Off Campus Form (to be completed and returned to the front office. By signing this form, parents/guardians give their child(ren) permission to walk off the CAV campus after school is out)

26-27 Equipment and Connectivity Request Form If you are in need of a Chromebook and/or assistance with internet connectivity for the purposes of completing assigned school work, please fill out the 2026-2027 Equipment and Connectivity Request Form. Please fill out the form once for each student that requires equipment.
