If you need to complete any forms, these can be found in your Welcome Email and at the MSP Next Steps for Enrolled Students page, linked in the Welcome Email and at the bottom of this slide.
All Students should submit their:
Consent Form (required if you registered by phone or email only)
A reminder that if needed, students should also submit to saic-youth@saic.edu:
Information about your child regarding additional support that they receive in their school classroom that can be shared with instructors to set them up for a successful experience at SAIC
Medical forms must be submitted each term even if the student is returning.
Additionally, students seeking disability accommodations are advised to contact the Disability and Learning Resource Center, or DLRC, to schedule a time to meet with one of their staff to discuss all of your accommodation needs.
SAIC is committed to providing opportunities for full participation in all programs for students with disabilities, including Continuing Studies students and Students At Large. Disabled students should first contact the DLRC to request reasonable accommodations. To plan for the most effective accommodations, we ask that you contact the DLRC at least two weeks before the start date for your course. The DLRC can be reached by phone at 312- 499-4278 or email dlrc@saic.edu.
If you have any other information regarding details that will allow our faculty and staff to set your student up for a successful experience at SAIC, please get in touch by emailing saic-youth@saic.edu.
Please note, SAIC may not be able to provide certain resources commonly available in primary and secondary schools. Reach out to our team and the DLRC with as much advance notice as possible to communicate any needs.
Be advised that students and families should contact the DLRC each time they take courses with SAIC. Accommodation communications may not automatically transfer over to a new faculty member.
Unescorted Dismissal Form:
MSP Families may also elect to submit an Unescorted Dismissal Form
Please note, students are not permitted to leave campus unescorted without prior written approval. Parents and guardians who do not give permission for unescorted dismissal will be required to park, enter the building, and pick their children up from the Sharp building lobby.
If you prefer that your child may meet you independently at the time of dismissal without checking out with an SAIC staff member, complete the Unescorted Dismissal Form. SAIC does not recommend this for any MSP students under the age of 11. Please note that unescorted dismissal status only applies to the time of dismissal, at 12 pm for morning classes in the fall and spring, or 4 pm for afternoon classes. Early pick-ups should be pre-arranged via email at saic-youth@saic.edu, and staff will escort your student to the pick up location. Please note that early pick-up requests must be scheduled for no later than 30 minutes before the regular dismissal time.
Students who do not have permission for unescorted dismissal will be released only to caregivers who present the student’s printed pick up pass, or a photo of it. Should you not have your pickup pass we can verify if you are on the list to pick up the student. You must have a state ID for us to check against our system. Caregivers will be asked to show the printed pick up pass or a photo of it when picking up a student for safety purposes. Caregivers will receive their student’s Pick-Up pass during Day 1 check-in inside the Sharp Building lobby. We highly suggest that you take a photo of this pass the moment it is issued to you.
