Submit, attached to an email, a properly formatted, 12 point font, letter using a rich text or plain text format. (Save as .rtf)

Completing a Job Application and Reference Sheet
Complete a job application and a reference list of at least three people to use for applying for a job.
The reference list should include each person's name, address, phone, email, and job title.
Begin to think about resumes and how your resume would look.

Read the form thoroughly so that you put relevant information in appropriate sections
Follow the instructions accurately (e.g. ink color, continuation sheets, block capitals)
