Cover pages are only required at the discretion of the instructor. Consult your instructor to determine whether or not they require a cover page. If they do, use the format below. The cover page is not considered part of the numbered pages. The video below the sample cover page will demonstrate how to format the header so the page numbers start in the pages following the cover page.
