Wingra - Referrals

OB-related diagnostic U/S - Use "OB Visit" smartset to order so that the order is formatted for the correct location (where the patient will delivery). Tell your MA order has been placed as they will need to fax the order to the appropriate facility.

Radiology-based referrals - Shereen in X Ray. If it is imaging study that cannot be performed in clinic (e.g. CT, MRI), still ask the patient to go up to X Ray if they have time because Shereen can schedule them. Otherwise, she has to call them to schedule the study, which is time-consuming as patients can be difficult to reach.

PT: Printed referral form required. Cross out listed facilities if patient has GHC or Dean Insurance and have MA make a copy of PT facilities that apply. If patient does not have insurance, you can refer them to MEDiC free PT clinic, which sees patients on the second Saturday of each month. See this site for more information regarding the MEDiC clinic. Referral to formal PT through Community Care (see below) is another option for patients without insurance.

Subspecialty referrals - Ask patients to go to the checkout area after their appointment as one of the Community Resource Specialists (CRS's, aka "registrars") can help them to schedule their appointment

Community Care: UW Health program that provides specialty care and advanced imaging studies for patients without insurance. If you anticipate that a patient will need Community Care for a referral, note this in the referral order in Epic and have the patient stop by check out on their way out to start the Community Care application with our CRS staff
