Verona Nursing Home Visits: end of 1st year
Seeing Patients at the Nursing Homes
R2s and R3s are scheduled for ½ day of nursing home monthly and see patients at Four Winds Manor in Verona and at Ingleside Manor in Mt. Horeb.
Patients newly admitted to the nursing home are seen every month for 3 months and then every 60 days after that. If a nursing home patient is hospitalized they must be seen every 30 days for 2 months.
Each month, the COA/Clinic Scheduler will give you a paper copy of the patients you are to see that month and they will also be listed on your schedule in Epic.
When you are scheduled to go to the nursing home, unless you are emailed otherwise, meet at Four Winds Manor at 1:30 PM.
Write a brief note in the paper nursing home chart and any new orders.
To document the encounter in Healthlink:
The encounter should be put in your Epic schedule. If it is not, open new “off-site facility” encounter with the chief complaint of “nursing home."
Vitals: File the most recent vitals in the doc flowsheet.
Progress note: Use the off-site facility Smartset.
Follow-up: In the box where it says “send chart upon closing”, send to Kris Kuhn so she can send the finished note to the nursing home.
LOS/billing: use the “off-site visit” smart set
For new patients, bill as new patient
For established patients, use 99307 for stable, 99308 for minor complaint, and 99309 for new problem
