NE Nursing Home Visits

R2s and R3s are scheduled for ½ day of nursing home every 1-2 months. R1 residents rarely see nursing home patients until the end of the 1st full year of residency.

Patients newly admitted to the nursing home are seen every month for 3 months and then every 60 days after that. If a nursing home patient is hospitalized they must be seen every 30 days for 2 months.

• Each month, you will be provided a paper copy of the patients you are to see that month and they will also be listed on your schedule in Epic.

• We round on patients at two nursing homes - Belmont and Karmenta. Everyone usually starts at Belmont at 1:30pm (regardless of whether or not you have your own patients to see there)

• Ask the nurses at the nursing home to print out vital signs and the most recent weights.

• Write a brief note in paper nursing home chart and any new orders.
