NE Patients on Chronic Anticoagulation (INR Patients)

At Northeast our INR patients are monitored and followed by the NE Pharmacist. Patients will work directly with the pharmacist for adjustments as needed. Your responsibility will be:

• Patients who are starting chronic anticoagulation for any reason (atrial fibrillation, venous thromboembolism, etc.) and require INR monitoring, can be enrolled in the Northeast Clinic INR program, which is managed by the Pharmacist at the Northeast Clinic.

• To enroll a patient in the INR program, type the dot phrase .DVNINR in a telephone encounter, fill out all the questions, and forward the message to: p dne – pharmacist.
