NE Diabetes and Hypertension Patients

Diabetes/hypertension registry patients follow-up every 3-6 months. The clinical staff have a virtual workbench that they use for routine outreach and reminders for our diabetic patients.

Diabetes Education

Maureen Schlager, RN, CDE, a diabetes educator, joined our clinic one day a week to help with diabetic education. Some of the RN staff can do glucose meter and insulin teaching and will help coordinate with the diabetic educator. Consult "Diabetes Education" to refer patients to Maureen.
