Huddles

Here at Northeast we use the Term “Rawhidem” for our huddles in the AM session and the PM session. With the huddles you review your schedules with your nurse and plan for your session.

R: Room set up (what will you need? Pap? Breast exam ECT.)

A: ACT or ASQ or Assess Falls

W: Who is your POA? Is POA in chart?

H: Health Maintenance needs: Tests, ECT.

I: Immunizations

D: Diabetes & Depression (print DM Flow sheet) (PHQ 2 or 9)

E: Education for the patient

M: Medication Reconciliation and Refills


Pre-visit planning

Look ahead in the chart review section of the patient and review last labs, look in Health Maintenance and see if they are due for any test, or vaccines.


ACO Metrics

Review nurse’s notes and determine if you need to address either the falls or depression. See handouts to refresh what you need to do.


Health Maintenance Do’s & Don’t

  • 0-17 yr old immunizations should not be postponed

  • Immunizations should never have overrides placed on them, all the information needs to be put in through historical Immunization/Injection

  • Postpones should not be greater than 1 year for all topics, please adjust below postpones greater than 1 year.


Medication Reconciliation

Please read your nurses notes in the documentation section. They will be advising which medications need to be removed from the patient’s medication list. They cannot remove any prescribed medications except antibiotics that have past the end date and over the counter medications. Please verify with patient that the medications that are pended are truly what they need. Review medication list with your patients. Please do not depend on your nursing staff to manage your patient’s medications.


Expectations of Nursing Staff

Nurses request that you work directly with your nurse. If your nurse is not at her desk and you find that you need something, please leave her a post it notes, If another nurse asks to assist you that is fine as long as your nurse is aware. Pleases read your nurses notes! Nurses know your patients very well and they can help you with many things. They are an excellent resource!


Reach Out and Read

Northeast has been a long time program of Reach Out and Read. This programs utilizes books in well-child visits with children. Susan Golz, ROR program coordinator, will provide you training so that you can utilize books within your visits.
